Avaliação neuropsicológica em indivíduos com transtorno da personalidade Borderline

Abstract

As alterações neuropsicológicas no Transtorno da Personalidade Borderline (TPB) são demonstradas na literatura e relacionadas a disfunções neurobiológicas. Entretanto, os resultados dos estudos não são coerentes entre si, e poucos estudos realizaram um levantamento global das funções neuropsicológicas dos indivíduos com TPB. Portanto, tornas-e necessária a investigação neuropsicológica no TPB. Esta dissertação está composta por dois estudos, apresentados em formato de artigos. O artigo teórico teve por objetivo realizar uma revisão da literatura sobre aspectos neurobiológicos e neuropsicológicos associados ao TPB, discutindo criticamente os principais achados empíricos e limitações metodológicas. Já o artigo empírico investigou, através de uma bateria neuropsicológica flexível e avaliação clínica, diferentes funções cognitivas em uma amostra de 21 participantes com TPB. Além disso, utilizou-se de uma tarefa de Stroop Emocional para verificar um possível viés atencional para palavras de valência emocional negativa de relação semântica com abandono e rejeição. Os resultados evidenciaram sintomas clinicamente significativos de impulsividade, ansiedade, estresse pós-traumático e experiências traumáticas infantis nos indivíduos com TPB. Os resultados da avaliação neuropsicológica demonstraram marcado prejuízo em funções executivas. A partir dos resultados são discutidas interpretações teóricas.

The neuropsychological alterations in Borderline Personality Disorder (BPD) are shown in the literature and related to neurobiological dysfunctions. However, the results of the studies are not coherent among themselves and few studies performed a throughout assessment of the cognitive functions of BPD individuals. Therefore, it becomes necessary to carry on the neuropsychological investigation of BPD. This dissertation is comprised of two studies, presented as papers. The theoretical paper aimed to conduct a literature review of the neuropsychological and neurobiological aspects related to BPD, critically reviewing the main empirical findings as well as methodological limitations. In the empirical paper, a flexible neuropsychological battery and clinical assessment instruments were employed to investigate cognitive functions in a sample of 21 participants with BPD. In addition, it was used an emotional Stroop task to verify a possible attention bias toward words of negative emotional valence in a semantic relation to abandonment and rejection. The findings showed clinically relevant symptoms of impulsivity, anxiety, posttraumatic stress, and childhood traumatic experiences among BPD individuals. The outcomes of the neuropsychological assessment pointed to a marked impairment in executive functions. Based on the results, theoretical interpretations are discussed.
