Abstract: Objetivos: Medir a prevalência da posição inadequada do tubo orotraqueal, estabelecer a freqüência do uso de fórmulas ou recomendações para estimar a posição do tubo traqueal e comparar a precisão dos principais métodos utilizados para estimar esta distância em crianças. Métodos: Incluídas todas as intubações via orotraqueal ocorridas em duas UTIP de hospitais universitários de Porto Alegre, entre agosto de 2004 e julho de 2005. Foi feita revisão do prontuário e entrevista padronizada até 72 horas após o procedimento. Foram avaliados o gênero, idade, peso, estatura, superfície corporal, motivo da intubação, comprimento do esterno e distância oro-trago-furcular. Obtivemos informações sobre os métodos de escolha do diâmetro do tubo e para estimar a posição do tubo traqueal. Através da radiografia de tórax, era definida a distância orotraqueal como sendo adequada, baixa ou alta. Os dados foram comparados pelos testes t de Student, Mann-Whitney, Qui-quadrado e correlação linear. Resultados: Foram incluídas no estudo 208 intubações realizadas pela via orotraqueal. Em 65% (135/208) constatamos que a posição do tubo orotraqueal (TOT) era inadequada. Em 183 das 208 intubações orotraqueais (88%) foi possível realizar a entrevista em tempo hábil (<72 horas) com o médico responsável pelo procedimento. Em 45% (83/183) destes procedimentos foi empregado algum método para estimar a profundidade de inserção do TOT. O uso de fórmulas ou métodos não se associou a uma maior taxa de acerto da posição do TOT. Peso, altura, superfície corporal e comprimento esternal apresentaram forte correlação com a distância orotraqueal. Em simulação realizada para comparar o desempenho das fórmulas que utilizam estas medidas, a maior taxa de sucesso obtida foi de 43,5%.Conclusões: A freqüência de acerto da profundidade de inserção do TOT é baixa. O uso de métodos para a sua estimativa não parece estar associado a um maior índice de sucesso.
Objectives: Evaluate the prevalence of incorrect orotracheal tube position, establish the frequency of formulae or methods for correct positioning of orotracheal tube estimation and compare the precision of main formulae to predict the depth of insertion of orotracheal tube in children. Methods: All orotracheal intubations occurred in two pediatric intensive care units of Porto Alegre between August 2004 and July 2005 were followed. Data were collected reviewing patient charts and by interview realized up to 72 hours after intubation. Patients were characterized by gender, sex, age, weight, height, body surface, intubation indication, sternal length and oro-tragus-furcular distance. Information about the method for tracheal tube diameter chooses and estimation of depth of insertion of orotracheal tube was collected. In the thorax radiography after intubation, orotracheal position was defined as correct, high or low. Data was analyzed using student’s t test, Mann-Whitney, chi-square and linear correlation. Results: We verified that in 65% (135/208) the orotracheal position was incorrect. Only 45% (83/183) any method for depth of insertion estimation was used. The use of any method was not associated with a higher success rate of orotracheal tube position. Weight, height, body surface and sternal length were strongly correlated with the correct orotracheal distance. However, in a simulation realized to compare the formulae performance, the best success rate was 43. 5%.Conclusions: The frequency for achieving the correct depth insertion of orotracheal tube was very low. The use of any method for estimation the orotraqueal distance neither was not associated with a higher success rate.
