Title: Diferentes tipos de substrato dentinário podem influenciar a resistência de união em cimento resinoso autoadesivo?

Abstract: Este trabalho se propôs a avaliar a influência de diferentes substratos dentinários da resistência de união de um cimento resinoso autoadesivo. Foram pré-selecionados 50 molares humanos, sendo 25 contendo restaurações de amálgama e 25 contendo tecido cariado. Os dentes com tecido cariado foram desgastados em politriz com auxílio de lixa de carbeto de sílicio de granulação 100 até a remoção de tecido cariado, expondo uma dentina afetada por cárie. Os dentes contendo restaurações de amálgama seguiram o mesmo protocolo, até a remoção da restauração, expondo uma dentina com alteração de cor. Os dentes que não apresentaram dentina afetada por cárie ou dentina alterada pelo amálgama circundados por dentina hígida foram descartados. Foram realizadas tomadas radiográficas a fim de selecionar os dentes que apresentavam dentina remanescente de 2 a 3 mm. Os dentes selecionados tiveram suas raízes embutidas em resina acrílica autopolimerizável. Foram confeccionadas pastilhas de resina composta microhíbrida (Sinfony, 3M ESPE) conforme as orientações do fabricante. A cimentação com o cimento resinoso autoadesivo RelyX Unicem (3M ESPE) foi realizada em dentina úmida, mantida sob pressão de 0,8 kg e fotopolimerizada com luz halógena (3M ESPE) por um período total de 80 segundos. As amostras foram armazenadas por 24 horas em 100% de umidade a 37ºC. As restaurações foram incluídas com resina acrílica autopolimerizável e submetidas a cortes seriados, obtendo-se o formato de palitos. Os palitos foram examinados e divididos de acordo com o tipo de substrato dentinário que apresentavam: hígido, cariado e alterado por íons provenientes de amálgama. Os espécimes (n=24) foram submetidos ao teste de resistência de união à microtração na máquina de ensaios universal EMIC DL-2000 com velocidade de 0,5mm/minuto.Os resultados foram submetidos a Análise de Variância e ao teste de Tukey HSD. O substrato hígido apresentou o maior valor médio de resistência de união (7,58±2,95 MPa). O substrato alterado por íons metálicos apresentou um valor médio de resistência de união intermediário (6,43±4,46 MPa) não diferindo estatisticamento dos demais grupos avaliados. O substrato afetado por cárie apresentou o menor valor médio de resistência de união (4,21±3,1 MPa) diferindo estatisticamente do substrato hígido. O tipo de fratura foi analisado através de imagens obtidas em MEV e a falha do tipo mista foi predominante no substrato afetado por cárie (85%) e no substrato alterado por íons metálicos (100%) enquanto que no substrato hígido houve predominância de falha adesiva (54%). Os resultados sugerem que diferentes substratos dentinários podem afetar a resistência de união do cimento resinoso RelyX Unicem.
The purpose of this study was evaluated the influence of different dentin substrate on the microtensile bond strength of a self-adhesive resin cement. Fifty human molars were selected, twenty-five teeth with coronal dentin caries and twenty-five teeth with old amalgam restorations. Teeth with dentin caries were ground with #100 silicon carbide paper under running water until the removal carie dentin remaining a caries- affected dentin. Teeth with old amalgam restorations followed the same protocol until the removal restoration exposing a discolored dentin. Teeth without caries-affected or discolored dentin surrounded by sound dentin were discarded. Radiographs were taken of the teeth in order to select those with remaining dentin around 2 to 3 mm. The selected teeth had their roots included in self-cured acrylic resin. Blocks of microhybrid composite resin (Sinfony, 3M ESPE) were made according to the manufacturer´s instructions. Luting procedures with the self-adhesive resin cement (RelyX Unicem, 3M ESPE) were made in moist dentin with the composite resin disc kept with pressure of 0. 8 kg and it was light activated with halogen unit (3M ESPE) for a total of 80 seconds. Bonded specimens were stored in water at 37ºC for 24 hours.The restorations were included in self-cured acrylic resin and they were serially sectioned into sticks. Specimens (n=24) were submitted to microtensile bond strength on a universal testing machine EMIC DL – 2000 at crosshead speed of 0. 5mm/min. Results were evaluated according to Variance Analysis and Tukey test (α=0. 05). The highest bond strength mean was obtained with sound dentin (7. 58±2. 95 MPa). Discolored dentin obtained intermediate bond strength mean (6. 43±4. 46 MPa) without statistically significant differences. Caries-affected dentin showed the lowest bond strength mean (4. 21±3. 1 MPa) differing statistically from sound dentin. Failure mode were examinated with a scanning electron microscope. The predominant failure mode was mixed to the caries-affected dentin (85%) and discolored dentin (100%) whereas sound dentin was adhesive failure (54%). The results suggest that different dentin substrates can affect the bond strength of self- adhesive resin cement RelyX Unicem.
