Uso do AcciMap para análise de acidentes em sistemas sociotécnicos: o caso da colisão entre o PR-GTD E O N600XL

Abstract

Esse trabalho consistiu no uso da ferramenta AcciMap, proposta por Jens Rasmussen em 1997, para analisar a colisão em voo de duas aeronaves de grande porte no Brasil em setembro de 2006. O relatório emitido pela autoridade aeronáutica brasileira serviu como fonte de dados para a elaboração do mapa de fatores contribuintes do acidente, que vitimou 154 pessoas e é o segundo maior na história da aviação civil do país. Ao final da construção do mapa, foi possível visualizar como cada fator contribuinte se propagou no sistema ao longo do tempo, bem como suas interações, proporcionando uma interpretação holística do acidente. Ainda, verificou-se que a maioria dos fatores contribuintes apontados pela investigação encontravam-se nos níveis operacionais e organizacionais das empresas envolvidas, semelhante a outros estudos que empregaram o AcciMap. Isso pode indicar que, por mais que atualmente procure-se analisar fatores externos às organizações, a maioria dos relatórios ainda atribui a maior parte da responsabilidade aos operadores da linha de frente do sistema sociotécnico. Nessa análise, a colisão entre duas aeronaves novas e com tecnologia de ponta, e subsequente queda de uma delas, evidencia a complexidade dos sistemas atuais, reforçando a doutrina de que acidentes não ocorrem como consequências de atos individuais de pessoas, e sim, de interações e processos que envolvem ações e decisões de órgãos reguladores, empresas e pessoas.

This study consisted in the use of the AcciMap tool, developed by Jens Rasmussen in 1997, to analyze an inflight colision between two large aircraft in Brazil in September, 2006. The final report, issued by the brazilian aviation authority, served as the primary source of data for the construction of the contributory factor map of the accident, which resulted in 154 fatalities and it’s the second deadliest crash in the country’s civil aviation history. Once the map was finished, it made possible to graphically visualize how each one of the contributory factors propagated throughout the system over time, as well as their interactions, providing a holistic interpretation of the accident. Further on, it was verified that most of the factors appointed by the investigation were situated at the operational and organizational levels of the companies involved, like other studies in which the AcciMap was used. This finding may indicate that, even that nowadays there are efforts to analyze factors external to the organizations, most part of the final reports still put major responsibility on front line operators of the sociotechnical systems. In this analysis, the collision between two brand new and highly technological aircraft, and subsequent crash of one of them, highlights the complexity of systems, reinforcing the doctrine that accidents do not occur as consequence of unique acts of individuals, but due to interactions and processes that involve actions and decisions of regulatory bodies, companies and people.
