Hipertensão arterial em receptores de transplante renal na infância: prevalência e fatores de risco

Abstract

Hipertensão Arterial Sistêmica é fator de risco para morbidade e mortalidade cardiovascular, afetando 1 bilhão de indivíduos no mundo inteiro. Em crianças transplantadas de rim a prevalência aumenta significativamente sendo essencial conhecer as associações dos diferentes fatores de risco e hipertensão arterial. Este estudo consistiu de uma coorte retrospectiva e foram analisados prontuários de 171 pacientes transplantados de rim, entre 1981 e 2008. Após excluídos 21 pacientes pelos critérios estabelecidos, foram registradas variáveis demográficas e relacionadas ao transplante de cada paciente e relacionadas com a presença ou não de hipertensão arterial. Foram registrados para análise os dados dos meses 1, 2, 3, 6, 12 e 24 após o transplante. No início do estudo, 90 pacientes eram hipertensos (60%). A média de idade foi 9,91 + 4,29 anos. A prevalência de hipertensão arterial durante os 24 meses diminuiu progressivamente. Há uma associação positiva (risco) entre doença glomerular e negativa (proteção) entre doença urológica como etiologia da Insuficiência renal crônica prévia ao enxerto e hipertensão arterial em todos os meses avaliados. Houve associação positiva e isolada entre tratamento dialítico ou hipertrofia do ventrículo esquerdo prévios ao transplante e hipertensão arterial nos meses 1º, 2º e 3º e entre uso de prednisona e HAS no 12º e 24º mês.

Systemic arterial hypertension is a known risk factor for cardiovascular morbidity and mortality, affecting 1 billion individuals worldwide. In pediatric patients submitted to kidney transplantation the prevalence increases significantly and knowing the association between hypertension and its different risk factors is essential. This study is a retrospective cohort and we analyzed medical records of 171 kidney transplant recipients, between 1981 and 2008. After excluding 21 patients by pre established criteria, demographical and transplant related data were registered and related with presence or absence of hypertension in the months 1º, 2º, 3º, 6º, 12º and 24º post transplantation. Ninety patients were hypertensives (60%) at the beginning of the study. Mean age of renal transplant recipients was 9,91 + 4,29 years. The prevalence of hypertension during the 24 months decreased. There is a positive association (risk) between glomerular disease and negative association (protection) between urologic disease as etiology of renal insufficiency previously to the transplant and hypertension at all months evaluated. There were positive association between dialytic treatment or left ventricle hypertrophy previously to the transplant and hypertension at the months 1º, 2º and 3º and between usage of prednisone and hypertension at the 12º and 24º month after transplantation.
