| Abstract: | Backgroud: The old people are frequently committed by psychiatric disorders, with a predominance of depression, which compromises the quality of life of them and also of their family, therefore requiring a precocious diagnose to the institution of the adequate treatment. Objective: To test the effectiveness of a five-item version of the Geriatric Depression Scale (GDS-5) for the screening of depression in old people of community. People and Methods: Two hundred and twenty one older subjects were included. All the participants had a comprehensive geriatric assessment including a questionnaire of socio-demographic informations, the Mini Mental State Examination for cognition evaluation. The GDS-5 was compared with the clinical diagnosis according the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition by Portuguese version of the Mini International Neuropsychiatric Interview, for diagnose of major depression by a psychiatric. The sensitivity, the specificity, the positive and negative predictive values and the interrater reliability of the five-item scale were also evaluated. Results: In the whole sample, 27. 9% of the subjects were depressed. The Geriatric Depression Scale whit five-item (GDS-5) had a sensitivity of 93. 4 (84. 5-97. 9), a specificity of 81. 0 (74. 3-86. 6), a positive predictive value of 65. 5 (54. 6-75. 4), and a negative predictive value of 97. 0 (92. 4-99. 2), a positive likelihood ratio of 4. 92 (3. 54-6. 84), and a negative likelihood ratio of 0. 08 (0. 03-0. 21). The GDS-5 showed a significant agreement with the clinical diagnosis of the depression ( = 0. 673), and a good interrater reliability (=0. 84; 95%CI 0. 80-0. 88) and test-retest reliability (=0. 79; 95%CI 0. 74-0. 83). The area under the ROC curve was 0. 94.Conclusions: The GDS-5 is an effective tool for screening depression in cognitively intact older subjects of community.
Introdução: Os idosos são frequentemente acometidos por distúrbios psiquiátricos, com predomínio da depressão, acarretando para eles e familiares, perda da qualidade de vida sendo, portanto o diagnóstico precoce na rede primária de saúde importante para a instituição do tratamento adequado. Objetivo: Testar a efetividade da versão de cinco itens da Escala de Depressão Geriátrica para rastreamento de depressão em idoso da comunidade. Pessoas e Métodos: Duzentos e vinte e um idosos foram incluídos. Todos os participantes tiveram uma avaliação geriátrica incluindo um questionário de informações sociodemográficas, o Mini-Exame do Estado Mental para avaliar a cognição. A GDS de cinco itens (GDS-5) foi comparada com o diagnóstico clínico de acordo com os critérios do Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition através da versão em Português do Mini International Neuropsychiatric Interview, para diagnóstico de depressão maior feito por um psiquiatra. A sensibilidade, a especificidade, os valores preditivos positivo e negativo e as confiabilidades entre observadores e intra-observador também foram avaliados. Resultados: Na amostra estudada 27,9% dos sujeitos estavam deprimidos. A GDS-5 mostrou sensibilidade de 93,4 (84,5-97,9), especificidade de 81,0 (74,3-86,6), valor preditivo positivo de 65,5 (54,6-75,4), valor preditivo negativo de 97,0 (92,4-. 99,2). A GDS-5 mostrou muito boas confiabilidades enter-observadores (κ= 0,84) e do teste-reteste (κ= 0,79). Área sob a curva ROC foi de 0,94.Conclusões: A GDS-5 é efetiva para rastreamento de depressão em idosos da comunidade com cognição preservada. |
