O efeito do uso da sucção não nutritiva com chupeta na apnéia da prematuridade

Abstract

OBJECTIVE: To evaluate the effect of nonnutritive sucking (NNS) with pacifier in premature infants very low weight in a neonatal intensive care unit (NICU).METHODS: we have followed forty four premature infants ≤ 32 weeks and with birthweight < 1500g, after 7 days of life, not requiring mechanical ventilation or continuous positive airway pressure. After obtaining informed consent from the parents, the babies were randomized into two groups: with pacifier offered during the period in the neonatal intensive care unit (group 1), and without pacifier (group 2). In group 1 the pacifier was recommended to be offered during gavage feedings and when the infants were awakened. The information was later obtained through the records of the monitor, filming and evaluation of results of polygraph records. RESULTS: The study groups showed very similar characteristics at the entrance to the study, however, there were no statistically significant differences between the two groups was not showing changes in breathing patterns of preterm infants evaluated by comparing the use or not of non-nutritive sucking using a pacifier. CONCLUSION: The results suggest that non-nutritive sucking with pacifier use does not alter the breathing pattern of preterm births in a population who frequently uses surfactant, early continuous positive airway pressure and xanthine, as well as other benefits have not been shown in relation to weight gain and gastric residue.

OBJETIVO: Avaliar o efeito da sucção não nutritiva (SNN) com chupeta em recém-nascidos prematuros de muito baixo peso numa unidade de tratamento intensivo neonatal (UTIN).MÉTODOS: Acompanhamos quarenta e quatro recém-nascidos prematuros com idade gestacional ≤32semanas e peso de nascimento <1500g, após 7 dias de vida, não necessitando de ventilação mecânica ou pressão positiva contínua nas vias aéreas. Após obtermos consentimento informado, os recém-nascidos prematuros foram randomizados em 2 grupos: com chupeta, oferecida durante a internação na unidade de tratamento intensivo neonatal (grupo 1) e sem chupeta (grupo 2). No grupo 1 a recomendação foi oferecer chupeta durante a dieta por gavagem e nos momentos em que os pacientes estivessem acordados. As informações foram obtidas posteriormente através dos registros do monitor, avaliação das filmagens e resultado do registro poligráfico. RESULTADOS: Os grupos estudados apresentaram características bastante semelhantes na entrada ao estudo, entretanto, não houveram diferenças estatisticamente significativas entre os 2 grupos não se evidenciando modificações nos padrões respiratórios dos prematuros avaliados, comparando a utilização ou não da sucção não nutritiva com o uso da chupeta. CONCLUSÃO: Os resultados sugerem que a sucção não-nutritiva com o uso da chupeta, não altera o padrão respiratório, numa população de prematuros que freqüentemente utiliza surfactante, pressão positiva contínua nas vias aéreas e xantinas precocemente.
