| Abstract: | A tese trata da doença mental como uma doença que através da história ocupou a atenção dos estudiosos da área médica, e que segregou e estigmatizou seus portadores. A concepção de loucura, exclusão, e a inclusão dos portadores de doença mental, trabalhados por Michel Foucault marcaram os estudos e as políticas de saúde como respostas científicas, sociais e médicas oferecidas pelas sociedades no decorrer das últimas décadas. Foucault contrapõe a visão e o tratamento social dos portadores de transtorno mental às categorias analíticas de Poder e Saber o que em muito tem contribuído para o desocultamento de preconceitos e estigmas para com os portadores de sofrimento psíquico. Nesta tese a Declaração de Caracas, a III Conferência Nacional de Saúde Mental e a lei Federal nº10216 (a Lei da Reforma Psiquiátrica), como documentos oficiais que regem as políticas nas sociedades ocidentais, são submetidas à análise de conteúdo sob o enfoque das concepções de Foucault sobre loucura, exclusão e inclusão na sua vinculação com o poder e o saber médico e científico. O objetivo da tese é fazer um estudo minucioso de três documentos oficiais considerados marcos importantes na atual Política de Saúde Mental e já apontados acima e a partir deles identificar se naqueles documentos o portador de transtorno mental continua sendo vítima de exclusão ou sujeito incluído socialmente e, se a loucura aparece ainda vinculada ao estigma e à intolerância daqueles ditos normais, ou se é concebida como uma doença passível de tratamento.Ao final do estudo foi possível concluir que, desta data até os dias atuais o processo da Reforma Psiquiátrica não acabou até o momento com a cultura de exclusão social a qual o portador de sofrimento psíquico tem sido vítima, mas inegavelmente trouxe uma possibilidade e uma capacidade transformadora principalmente no estabelecimento e relações de solidariedade entre loucos e não-loucos, em que todos os sujeitos podem e devem ser vistos de forma inteira, singular e cidadã. Apesar da exclusão ser uma categoria que ainda perpassa o mundo da doença mental, os avanços tanto na área científica como na legislação, trouxeram mudanças radicais no conteúdo formal dos documentos analisados e simbolizam um novo saber construído ao longo dos doze anos de luta para a aprovação da Lei Federal nº10216, assim como apontam para uma atenção baseada em princípios comunitários e participativos desenvolvidos em serviços abertos, situados no território em que o portador de sofrimento psíquico vive numa clara superação do modelo hospitalocêntrico vigente de forma absoluta até aos anos noventa e num processo de ruptura cotidiana desta data até os dias atuais.
This thesis deals with mental disease as an ailment which through history has occupied the attention of the medical area scholars, and has segregated and stigmatized its carriers. The concept of madness. exclusion and the inclusion of a mental disease carriers, worked out by Michel Foucault have marked the studies and the health politics as scientific, social and medical answers given by societies in the last decades. Foucault counterpoints the view and the social treatment of those carrying a mental disturbance to the analytic categories of Power and Knowledge which have contributed a lot on revealing prejudices and stigmas towards those who carry a psychic suffering. In the present thesis the Caracas Declaration, the III National Conference on Mental Health and the Federal Law n. 10216 ( the Psychiatric Reformation), as official documents which rule the west society politics, are submitted to a content analysis under the Foucault concepts about madness, exclusion and inclusion in their connection with the medical and scientific power and knowledge. The aim of the present thesis is to make a detailed of the three official documents considered as important landmarks for the present Mental Health Politics and already pointed above, and from them identify whether in those documents de carrier of a mental disturbance is being treated as a victim of exclusion or a socially included subject, and whether madness appears still linked to the stigma and intolerance of those said normal, or whether it is conceived as a disease liable to treatment.At the end of the study it was possible to conclude that from this date up to the present day the process of Psychiatric Reformation did not end to this moment with the social exclusion culture of which the cariier has been a victim, but undoubtedly brought a possibility and a transforming capacity mainly in the establishment and a solidarity relationship between mad and non-mad people in which all subjects can and must be seen in a whole, singular and citizen-like way. In spite of exclusion being a category which wtill prevails in the world of mental disease, the advances both on the scientific area and on the legislation, have brought radical changes for the formal content of the analyzed documents and the symbolize a new knowledge built along the twelve years of the struggle for the approval of Federal Law n. 10216, as well as they point tosome attention based on community and participating principles developed in open services, located on the territory on which the carrier of a psychic suffering lives in a clear overcoming of the hospital-centered model prevailing up to the 90”s as well as in a daily rupture process from this date up to the present days. |
